猜想翻译过医疗器械的同仁一定经常见到以下一句话：
Federal law restricts this device to sale by or on the order of a physician.
似乎有两种理解：
一种是此装置只能根据或遵照医嘱销售，即只有在医师有订单时，才能销售，而且只能是订什么你销售什么。
另外一种是此装置只能由医师或遵照医嘱销售，即医师在销售器械。
请教美国同仁，美国的医生可以卖医疗器械给患者吗？如果在美国是这样的，直接译成中文，放在中国的环境下似乎也不妥，因为中国目前在整顿医生乱开药和特殊检查的行为。

小弟既非住在美国，也不是医学中人，不过单凭句子结构本身来看（by or on the order of a physician），还是觉得应当择第1个版本。至于到底是谁在sale，则没有点明，反正他或她就按照或者遵照physician的指令sale。
但愿不会添乱：）

再说了，医疗器械（家用保健器械除外）这东西也不大可能向个人销售。从这个角度讲，这句话在整篇文章翻译成汉语之后就变成了废话。

第1个版本也是我的理解，我以前一直是这样翻译的，只是后来有人（backeditor)提出异议，我才怀疑自己的理解是否正确，才想向人在美国的同仁请教。
有时这些法律语句实在叫人费解。（^_^）

单独看，sale by doctor’s order 或 sale on doctor’s order，都还好明白；但凑在一块就不很明白了。
不过，“to restrict to sale by or on the order of a physician” 总的看，应该只是“无医嘱不得销售”。

至于"by the order of a physician"与"on the order of a physician"到底有多大区别？我认为没多大区别，只是起草文件的律师为了保险起见有意罗嗦一些而已。
但是，不应理解为"Federal law restricts this device to sale by a physician"。如果法律规定只能由医师销售，那么医师到哪里去买？;)

IMO the sentence could be rewritten as follows:
Federal law restricts this device to sale by a physician or on the order of a physician.
So I think it refers to either situation. The device could be directly purchased from the doctor's office, or it could be acquired from a drug store(or medical equipment store) with a doctor's prescription.
The following paragraph doesn't use the exact same verbiage, but with the extra commas inserted, is very clear on its intended meaning.

Here's another link that has the same statement as quoted by Lu Zou.

As could be seen, the statement sometimes could apply to medical supplies/devices as small as a nebulizer.
I very seldom go see a doctor. But from my limited experience, not too many physicians in the U. S. do direct sales to their patients. A few doctors may carry some hard-to-acquire items purely out of consideration of the convenience of their patients.

二、拙译：美国法律规定，此装置只能由医生出售或遵照医嘱销售。

遵照医嘱销售：sold (by manufacturers, etc.) according to the instruction/order of a doctor

这句话还是有点语病，和原文的 ambiguity 有异曲同工之妙。

这里我最喜欢的是姚兄的译法。 But may I also suggest that "order" here 是订购,不是医嘱。
If something is "on order" that means it will be delivered later. 是订购. Here, "order" is an instruction indeed, by the physician, which is not "GIVEN" to the patient, but to the medical equipment supplier. If the word "order" here were to have a verb to go with it, the verb would be "to place (an order)".
医嘱 is the type of instruction the doctor gives to a patient.
看咱来个偷梁换柱：
美国法律规定，此装置只能由医生出售或由医生订购。
Shang 的意译也很到位:
“该用什么，由医院/医生决定，病人不能在外边买了拿到医院去使用”。
