What's your specific problem with the form? Jan 2, 2006

Can you give any specific problem that you have filling in the form? I know it is quite a pain but is manageable and the help you can find online is actually really helpful (that is, if you find the right website).

Good luck,
Ingo

I just sent one off today, and am also an EU resident. It's pretty straightforward:
Part I
Line 1: Your name
Line 2: N/A
Line 3: tick individual (for me at least, unless you are any of the other entities it lists)
Line 4: Your address in the right boxes
Line 7: your tax number in your country of residence and therefore taxation (but I think it's optional)
Part II
Tick box a, and put the name of your country
Part IV sign and date

And that's it!

Hope I helped,

Carolyn
