Japan is divided into a number of prefectures (of which Hokkaido is not referred to as a "ken" = prefecture), Osaka fu and Tokyo to. Osaka and Tokyo actually include entire cities (shi), and Tokyo also has ku, divisions of cities that are translated as wards. So Tokyo is not strictly a prefecture. "To" is more or less a "metropolis".

(I was born and raised in Japan, by the way.)
