Taka formułka oznacza, że zainteresowanym jest sam pracownik, którego zaświadczenie dotyczy.

"upon request" raczej oznacza "na żądanie" (czyjekolwiek).

Jeśli mam rację, to proponuję zmienić na " ...issued at the request of the employee".

"Employee" jest kolejnym problemem, bo w moim tekście pasuje. Gdzie indziej niekoniecznie.
Poradźcie coś, proszę.

ewentualnie proponuję "... issued at the request of the person concerned" - omija się problem pracownika.

Tak właśnie bym napisał, no chyba, że napisałbym "....issued at the request of the individual it refers to"
