If it's a medical term, I usually try my Zanichelli IT-EN paper medical encyclopaedia first (especially as by now I've noted down all sorts of useful additions and abbreviations in there). After that, Kudoz term search is my next stop.
If it's a general term, I usually try Garzanti IT-EN online first. If I get no luck there and it looks to be an obscure term, then De Mauro IT monolingual online normally tells me what it means so I can find an appropriate translation.
My other paper dictionaries and resources tend to be second or third choices.
So it's impossible to answer the question, as what I prefer depends on what I want to look up.
