Strange Formatting Issues

At my company we have Versions 5, 6.5 and 7, and with all versions we fairly regularly experience formatting issues.

This is with a variety of files, some just straight forward Word files with no images in. The problem is that often the formatting seems to get reversed once it is cleaned up, so that what was Bold in the source file and uncleaned Target file becomes not bold and vice-versa. It also changes fonts from Arial to Times to Courier seemingly at will. This is very frustrating having to go through and reformat whole documents once they are clean.

Can anyone help us? What are we doing wrong?

It usually helps if the styles are defined within the document. Trados seems to respect these and, if it does not, you can update the file once it is cleaned fairly easily by reapplying the styles.
