Though the authentic term may sound way cool - "inisyal sa munang titik ng gitnang pangalan". I doubt it would even be used in a form. FYI, an article in Tagalog published by the prestigious De La Salle University, Manila even opts for the borrowed English "middle intial".

Middle Initial in Europe hindi na nila isinusulat or ginagamit unless they use/inicate the complete middle name.
Sa Philippines and North US they use Middle initial as the initial letter of mothers' surname.
Sa mga Tanggapan Pribado o Goberno and/or School they define middle name as apelyido sa ina at ang middle initial ay panimulang (Starting letter/initial) letra ng apelyido sa ina.
Anyway it depend to someone or translator how he/she prefer to use the sentence or phrase.

I mean not only define but they also use this when they give an instruction kapag nagfile ka ng papers sa mga Private and/or Government Agencies and in a School.
And when you use panimulang letra ng apelyido sa ina understood immediately.

Explanation:
middle name is gitnang apelyido .
apelyido is surname or last name.
pangalan is given name.

you can use 'inisyal' or 'unang titik' (that will be very Filipino) ng gitnang apelyido. However, you need to know that even in the Phils. almost all government forms are in English except for the only one I have at hand. If you are looking for exact equivalent for M.I. you can't find any. You may still need to use M.I. otherwise imagine how lenghty it will be in your translated form.

The form asked for middle name not middle initial though.
