if it\'s for a longer time (ie several weeks, months) then a \"request for a leave of absence\" would be better.

agree  louisajay: BUT - a request for holiday doesn't mean you are going on holiday. In the UK at least, we call the annual allocation of paid leave days "holiday", whether you are going on holiday or not.
