Creo que este es el concepto más claro. La ref. te muestra el modelo de form que se firma.

Aunque también he visto por allí \"info médica protegida\", por ej en:

aunque no creo que se la opción más feliz en castellano.

disagree  María Diehn: "datos personales de salud" sería una buena traducción. "Autorización para utilizar y divulgar datos personales de salud" es un concepto diferente.
