-> Een 'status' in ieder geval niet, dat is een map waarin je de gegevens van de opgenomen patiënt bijhoudt. See for 'patient's chart', for instance, the description in the discussion box.

disagree  Barend van Zadelhoff: I assume it should be kept in the patient chart (the ‘status’ (chart) differs from the ‘dossier’ (file)) for immediate availability. It will be removed when the trial is completed.// See D-box for 'status'. Thank you in advance for considering.

-> Thanks. It's dossier (a file), as per van Dale.

agree  Textpertise: The word "status" in Dutch actually means case history or anamnesis. It can be either a chart or a dossier. A chart is usually used to record the current treatment and the history would usually be in the file. See van Dale on Status.

retain in patient's file

Explanation:
I think this would be the way it would be said in the UK. Of course, it is just another alternative for the answers already given but I feel "retain" is more likely to be used in this context than "enclose" and it is just a bit more formal than "keep" ;-)

Reference information:
Physicians, too, have several concerns regarding the informed consent process. “What we hear from physicians is they want access to the forms. Physicians want the information at their fingertips without having to ask other people to go around and look for it. And they want to know before they start a procedure if this information [a signed consent form] is in the [patient’s] chart or not,” says Sam Rubinsztain, vice president of product development at gMED (General Medical Applications).

The case law and rules pertaining to informed consent have changed over the years and all 50 states now have legislation that requires some level of informed consent.

Several of the common elements required for full disclosure have been summarized by the American Medical Association (Table 1) and other groups representing specialists or quality assurance organizations. [AMA 1998] For example, federal regulations spell out the minimum requirements for a properly executed informed consent form (Table 2) and state that this form must be in the patient’s chart before surgery.
