disagree  OTMed (X): za pozwoleniem, ale nie. Patient Report to raczej formularz badania pacjenta niż Karta Wypisowa zawierająca info dot. diagnostyki, leczenia, rozpoznanie, zalecenia etc. To zresztą również wynika z linków, które podajesz.
