your proposal is grammatically correct, but it reads a bit awkward. especially the latter part "of werk deze bij" = "or update this/these", where it's not clear whether "deze" refers to "informatie" or "velden". comes down to the same, but I don't really like this way of formulating a sentence in Dutch.

My alternative suggestion means:
If necessary, you may complete or update the information in the fields below.
