psychotic because she might consider him as threatening her assumptions

Explanation:
That's how many think of others as mad - they threaten their ways of seeing things and here "she" might be feeling threatened in that way.

neutral  Bernhard Sulzer: that means he could really be psychotic. But we're to think she could only think he's psychotic in the sense/as in... (a non-medical "way"), so it's believed he's not really "psychotic". My thoughts.

except if (she) was using the word in its general not medical/clinical meaning

Explanation:
"except in the the sense that" is being used to contrast the medical/clinical term psychotic with the more general or literal use of the term mad.

When you say to someone, "you must be mad to even think that," you are using the term mad to be mean thinking in a dangerous way. That dangerous thinking could be as described, as it is in your text, as challenging a way of thinking or it could be thinking in a way that reflects a serious misunderstanding which misunderstanding could be harmful.

"except in the sense that" in your context, and in general use, refers to the meaning of a word or phrase which may commonly mean one thing, but could also mean something quite different. Sense here refers to "meaning in which it is" this not that.
