Surrogate Authorization Form

I ______________________________ (print name), the mother/father/guardian (circle one) of __________________________________ (patient), hereby authorize
______________________________ (print name) to visit my child in the (circle one) Neonatal Intensive Care Unit, Pediatric Unit, Pediatric Intensive Care Unit when I am not present.

Can anybody tell what does the term "Print Name" mean? Also, is there any equivalent term for "Surrogate"? It is very hard to translate "Surrogate" into Urdu.
