English term or phrase: I lose track of were I am going
Does this imply forgetting the destination of the journey (of a drive in this case) - or losing the proper or planned route that leads there?

This is taken from a psychological questionnaire on driving, so I can provide no further context - except for the fact that there is another questionnaire item there, "I forget where I am driving to," which seems to cover the destination bit.

closer to "forget where I am driving to" but can also mean "lose one's way"
Explanation:
The idea of "getting off track" is straying, deviating from the only route one should take (physically, mentally, psychologically, or emotionally) "Losing track of" implies letting one's mind wander and forgetting the main object of what one is doing.

i lose track of were i am going
see explanation

Explanation:
It's that feeling when you are going somewhere and for a few moments you forget where you are going and why.

Sometimes on psychological questionnaires they will ask you the same question in different ways, to account for questions being interpreted differently according to how they are worded.

agree  Stephanie Ezrol: I think you raise an important point about the context - that they ask the same question with different wording. Both questions certainly imply destination.

agree  Sheila Wilson: letting one's mind wander and forgetting the main object of what one is doing - I think that sums it up very well. In this case, letting the car follow the road - might be the right road but you might have missed a turning while you were in a dream
