How should I understand "as first expectancy is filled out"?

It is usual for a doctor to issue a sick note with an expected date for return to work. I suspect that this is about changes to this date with subsequent sick notes, or even if the employee returns before the given date.

Explanation:
My interpretation is that the above phrase means that the number of calendar days on sick leave due to an incident will change when the employee who fills out his/her form for sick leave occurs. The person may take 3 days off for sick leave, or even 7 working days, and depending on the number of sick leaves taken, the total annual or monthly number will change according to how may days that employee has used for sick leave. And the counting will start when that employee uses the sick leave for the first time.

And, of course, needless to say, sick leave must be approved by the employee's supervisor or boss.
