English term or phrase: Consent presenter cards
A presentation of a clinical study. They say it is not a substitute for the informed consent document. In a different place, they call it "desktop presenter cards".
Can you please explain what they are? If I know what they are, I will be able to find something appropriate in my target language (hopefully)!

Explanation:
Consent presenter cards: are cards/forms, which are given to the next-to-kin or guardians (presenter) of a patient to give consent when checking in the patient.

Informed consent document: Is a form after an examination has been done, and an informed decision about the illness is decided, and they need the consent of the patient/guardian to carry out further treatment or surgery.

Desktop presenter cards: are the tear off booklet cards at the front desk when you go into a clinic/hospital where you give your basic details, name, address, health care provider, next to kin, blood type, Health insurance company etc..
