This may be far fetched, but if the original language was an Asian language, such as Chinese or Japanese, this could be a bad machine translation. The "fly out" part could be "jump out", meaning to leave the form, but it could also be "send". As to the “mitigations have to be followed up”, "mitigation" could be a bad translation of the word "support", and this phrase may mean that a message window pops up saying the support staff will follow up, or alternatively, asking for confirmation whether the user wants the support staff to follow up.
I am not sure what the form is for, but if it is for any sort of feedback, or reporting purposes, the follow-up part would make sense.
Again, this is just speculation, based on many mistranslations I have seen, and on my guesses on what the original might have been. |
