agree  David Moore: The only possible difference is that you might well have an annual subscription to pay for a "club". For a fitness centre, you would normally pay on a per visit basis, I think

  -> Thanks David, I thought the same, but then decided that payment options - both might be offered by the same place - aren't that important.
agree  Ken Cox: a possible (but by no means general) distinction is that a health club is probably public or commercial, while a fitness centre could potentially be private, e.g. a room on the premises of organisation for the use of the employees or members, or in a home
