The second part of the request is a little bit tricky. A plaintiff usually requests the court to oblige/require a defendant to pay such plaintiff the claimed amount or to rule in favor of the plaintiff/ the plaintiff's requests.
In the above case, I believe it should be stated as follows: Upon establishing [the claim], to adjuge that the plaintiff is entitled to the claimed amount/ to adjudge the amount the plaintiff claims.
P.S. Though the word "claim" is not mentioned in the request but rather than implicitly implied in the context, it is always recommended to put such words between in brackets to make the meaning as clear as possible, as they are not part of the source text, especially in legal translations.
