Arabic to English translations [PRO]
Medical - Medical (general) / Consent Form

Arabic term or phrase: الحالات الاستثنائية

This is one of the sections of a form where by the patient is requesting for administration of an operation.

The complete line is :

الحالات الاستثنائية للعملية الجراحية او النبج ان وجدت هي (اذا "لا توجد" يرجىى الافادة بذلك) : ____

Is "unusual cases" correct?
"Unusual cases of the operation or of anesthesia, if are/were found, are (if “none found”, please report this):__________________________ "

I'm not quite clear on the concept. If it's the patient filling this out, how is he supposed to know of the unusual cases that were or will occur as a result? Based on conversation with the medical team? Or is the intention here completely different?

I would very much like to know what your professor thinks. Are you studying at NYU SCPS? BTW, is he a native speaker of Arabic. I'm asking coz the Arabic text looks like it was originally written in English and then Google translated :)

I agree with Dima that in this context "adverse reaction" seems to make more sense.

God bless Ctrl +. That's the only way to read this "miniature font".
The added context you provided is actually confirming my hunch that this is from a medical history / consent form.
I can't be %100 sure but I believe this is a question inquiring about "adverse events / reactions" that may have occurred after a prior operation or anaesthesia.
In my experience, Adverse Events is a term more popularly used in clinical trials as opposed to Adverse Reactions which is used in the medical practice field.
What do you think?

3. اطلب اعطاء البنج الذي قد يعتبر لازما او موصى به في راي الفريق المسؤول في المركز العيادي----- ولقد تم اطلاعي على خيارات البنج والمخاطر والفوائد المرتبطة به وانا افهمها.
5. اطلب من سلطات المركز العيادي ------ التخلص من انسجة او اجزاء قد تلزم ازالتها.
