Explanation:
Please note that 投保年龄 is not the age of the insured, but rather the age at which the insured enters into the insurance contract. In other words, if the insured is now 50 years old, but the age at which he enters into the insurance contract is 30 years old, the 投保年龄 is 30 years old rather than 50 years old. "Age of the insured" would mean 50 years old, which is wrong.

Despite being a Japanese site, the first reference contains absolutely the best definition of "age of insured" that I could find. The second reference contains a good example of "insurance limit" used in context.

"Entry age of insured" is, in fact, correct. However, after consulting with an insurance professional, she informed me that "age of insured" implies the age at entry, so it is not necessary to put "entry" at the beginning of the phrase.
"Age of the insured" is gramatically correct, but insurance companies generally just use "age of insured" in their policy documents.
