Two pension sytems exist in Taiwan. Moreover, those choosing the new pension fund sysem are eligible for another option ( 選擇勞退新制之勞工，其始得個人自願另行提繳退休金).


勾選參加制度（1.勞退新制2.勞基法舊制3.暫不選擇）：（如為適用勞基法之本國籍勞工，本欄位一定要擇一勾選填報）
請依勞工交回之意願徵詢表據實於1、2、3欄位勾記。

(一)勾選勞退新制者：請於「1.勞退新制」欄打，另「勞退月提繳工資」及「雇主提繳率」欄位一定要填寫，選擇勞退新制之勞工，其始得個人自願另行提繳退休金，並請依七、（二）說明於「勞工自願提繳率」欄位填入資料。

(二)勾選勞基法舊制或暫不選擇者：請於「2.勞基法舊制」或「3.暫不選擇」欄打，其餘欄位均無需填寫，且其個人不得自願提繳退休金。（※如單位所有勞工均選擇勞基法舊制或暫不選擇者，亦須申報送回，該項資料將彙整陳報行政院勞工委員會，作為勞基法舊制提撥勞工退休準備金查核使用。）
