"切結書"是台湾的说法，相当于我们平时说的“协议”、“合同”、“合约”。
“立切結書人”就是立约人。
立约人根据合同、协议的不同，可以有不同译法。
常见的译法是：Promisor;Contractor
如果是要求签字（签章），后面又没有单独的（签字/签章）的字样，我觉得译为“Signature”就可以。
另外，根据合同的不同，还可以译为：Applicant（即申请人）, Affiant
上面用大写是考虑到这段内容一般单独出现于签字处。如果是在句子中，当然没必要用大写。
