9日下午国妇婴建大卡。
预约的时间是下午1：30，1：00到的医院，医生还没有来。就在8-5门口坐着。
到1：30分左右的时候，大概排队有10来个人。然后来个建卡的医生。集中让我们做一些事情，如下：
1、确认胎盘的处置方法，（就是初检时候发的那个表格）
2、确认流产史。近视眼是否800度以上。还有一些内科慢性疾病史要写在初检发的流程表上。
3、确认是否有上海医保卡。
4、告知签字完后要挂号，建大卡一个号，口腔科一个号。
5、告知挂号之后检查尿常规，口腔，血压身高体重之后到8-5门前继续排队。并要求将身高，体重，血压的指数也写在初检发的流程标上。（一个MM没有写就给医生退回来，结果就排在后面了）
然后等签字的医生来了，让他在病例上面签字（昨天下午是个男医生～）。他会看过初检时所有的报告，然后在病例卡上面签字。拿着签字就可以去建卡了。建卡费用如下：
1、大卡挂号：14元 2、口腔科挂号：14元 3、尿常规：38元
然后先去了口腔科，医生关照你一些注意事项，要求我买漱口水，但是被我拒绝了。然后到产科护士台刷卡拿尿检的杯子试管（不是在大厅的护士台了，是在产科护士台了）。回来后量了血压体重。按照要求将数据写好，在8-5门口排队。估计在六个人左右的时候，那个建卡的女医生来了，给了我们一张纸头，上面写着要带的东西，有不少东西，我记不清楚了，但是真正要用的是下面一些东西：
1、结婚证 2、户口本 3、小卡（孕妇健康手册） 户口本有人没有带问题不大。
