Sorry, not much context. This is on a document headed 'Karta pojištěnce', concerning someone's health insurance. After various details - rodné číslo, name, address etc. - on the last line of this standard form there's 'Kapitace' followed by an 8-digit number and a name (not the name of the insured person): MUDr. xxxxxx.

capitation - capitated payment managed care

1. A method of payment for health services in which a physician or hospital are paid a fixed amount is paid per enrollee to cover a defined scope of services for a defined population set–aka covered lives for a defined period of time, regardless of actual number or nature services provided; capitation may be used by purchasers to pay health plans or by plans to pay providers;
2. capitation payment is made on the basis of the number of members each month.

the 8-digit number will probably be the doctor's registration number, and obviously the MUDr. xxx is the name of the doctor

Whenever a doctor sees a patient he/she receives a sum which is worked out by a complicated formula, which I have never had the time or inclination to work out :) It is about 50 to 70 Crowns as far as I am aware.

Asker: Thanks. This makes more sense than what I found in dictionaries, which gave me the vague idea that it's a kind of head-tax or poll-tax.
