Czech term or phrase: expozitura
Again, this is from the 'Karta pojištěnce'. In the top left, there's the heading 'Pojištěnec', and on the same line, on the right, there's 'Expozitura [four-digit number]' followed by 'Počítač [single digit]'.

neutral  Martin Janda: Dokud se to v 1 textu nepotká s pobočkou, je to OK. Ale jinak je to obvykle menší než pobočka, a v hierarchii o 1 level níž.

Reference information:
For your information (not in your context) - "pobočka" vs "expozitúra".
"Pobočka" is usually bigger and providing wider range of services than "expozitúra". They can be sometimes translated as "full-service branch" and "subbranch".
