Still determined to crack this. It is definitely "UK", not "UB" (thought of that). It is right at the top of the report, along with the date, the names of the referring physicians, etc., and so does not refer to anything as specific as meds, lab reports, procedures, etc. (that all comes later). I did find this on Aarhus Hospital'sl e-documenation instructions: Stuegang skrives løbende hele dagen, men der gåes sædvanligvis stuegang ved 9-tiden. UK-beskrivelsen markeres med UK. Hvis patienten skal overflyttes til anden afdeling, skal der medsendes kopi af journalen – flyttes ud som notat i journalen, så afdelingssekretæren er opmærksom på dette.

Is this a faxed copy? Could it be UB instead of UK? In that case, I would vote for 'udskrivningsbrev', as that is what is usually sent to the GP?

I know these medical records are often extremely fragmented, but is there any clue in the text?
E.g. what has the patient been treated for? That would give a rough idea of the medical field.
Does this expression come near the medication list, the physical examination, the list of scans, radiograms etc. taken?
Is there any reason why this point in particular would be drawn to the GP's attention?

Hope you can give us some hint without revealing confidential patient details....

Hi David. Don't think it can be "udkast". This is not a draft, it is part of the final package of reports sent to the referring physician by the hospital department where the patient died. No more reports (more complete, for example) will follow. Have you actually seen "UK" used as an abbreviation for "udkast"? I never have but, then, I haven't seen everything... Richard.
