My guess is that this is part of an attempt to reduce waiting lists and make treatment more consistent and therefore more beneficial to the patient.

These packages would be appointments and check-ups at regular intervals, following standard patterns, so that staffing needs could be calculated and planned.

A lot of treatment is unavoidably individual and ad-hoc, but with chronic conditions, allowing a little flexibility for individual adjustments, there is no need to ´invent the wheel´ every time.

Standard packages commit the health services to providing the necessary care, but are probably also connected to the need to economise wherever possible on staffing and resources, while maintaining the level of service to patients.
