In your sentence I think I would go with 'diseases, complaints and medications'.

I think ailment is somewhat old-fashioned, context dependent, as is 'afflictions'. Disorders are more complex or psychiatric. Aandoeningen are often symptoms, klachten, something that is bothersome, often longtime.
Alternatively, you could say 'diseases and symptoms' or 'ilnesses and complaints'.
