Dutch term or phrase: prestatiestaat

Hi all

I am translating a contract template for companies recruiting temporary skilled and unskilled workers in various sectors. The term above came up in the following context:

'De gebruiker dient wekelijks de prestatiestaat aan XXX te bezorgen, en dit uiterlijk op maandag volgend op de prestatieweek uiterlijk om 14 uur.'

Would 'performance sheet' or 'statement of services rendered' be an option? Thanks in advance for any suggestions!

Explanation:
A statement of services rendered is a bit more detailed, explaining not only when the work was done, but also what work was done and how, the cost of each service etc etc etc.

Performance relates more to an evaluation of the quality of the work.

I really think this is just a log record of the days/hours worked for the week, and perhaps a brief description of what was done. The outplacement bureau probably uses them to keep track of how much the temp is supposed to be paid, and so that there is a record he/she actually did something.

I have to keep these for several companies that contract me for hourly work offsite. And if they question my hours, then there is a record of what was done.

Just as well, prestatieweek is the week that the work was done (i.e. the week worked) not the "performance week".
