It's a quite common document within electrical engineering companies. I suppose the directive refers to the CE-label for machines, installations, equipment.

neutral  writeaway: I think you have the right context-ie machinery, not cars, but it has to be a certificate, not a declaration in English. it's like the one we get with our cars in Belgium-a declaration is a statement. this is a piece of paper.

Explanation:
The gelijkvormigheidsattest is the Belgian national type approval, the CVO (Certificaat Van Overeenkomst) is a European type approval. This Belgian attest can not be used for import inspection in the Netherlands, it is not the same thing.
