Prof. Tom Whatever, Ph.D. or Prof. Dr. Tom Whatever
Explanation:
In academic publications I have seen both. I would choose the first option myself in a British or US context, for example.

Prof. Tom Whatever

Explanation:
In English, you would only include the higher of the two qualifications (otherwise it would be like putting "BA MA").

disagree  Jack den Haan: Hmm... not so sure about that Phil. 'Prof.' is not an academic qualification, it's a title (at least here in the Netherlands). A further point is that you're not translating the "Doctor" part here (i.e. Doctor of Philosophy, abbreviated as Ph.D.)

agree  Sally Hill: Also according to the book "Righting English that's gone Dutch" by Joy Burrough.

agree  Lianne Van De Ven: Oh, I see, you entered it. I have to agree now :-) with the first, like Ron. I explicitly do not agree with the second, actually. Because Dr. is, or let's say, tends to be a medical doctor//I know Jack. But we're translating dr now, right...

  -> Thanks, Lianne :-) PS: In the NL, the doctor's title is certainly not restricted to medical doctors in an academic sense. A familiy physician, however, would usually be called a doctor even if he/she "only" has a Master-level qualification in medicine.

agree  CI95: agree with 1st, disagree with 2nd. I used to be an academic myself and don't recall ever seeing Prof. Dr. It was either one or the other. But I agree that not all profs have PhDs and that Dr is perfectly natural in the present context.

  -> Thanks, dr. CI95 ;-)
