Dutch term or phrase: arbeidsovk
This is taken from a list of instructions for HR personnel (new appointments).

Payroll volgende documenten bezorgen:
o kopie paspoort
o kopie loonbelastingverklaring
o kopie aanvraag arbeidsovk

I'm not ruling out a typo, but it could also be a weird abbreviation for arbeidsovereenkomst.
