Het is, lijkt mij, niet precies een aanvraagformulier voor ziektekostenverzekering. Misschien is dit een onderdeel van de aanvraag: verzekering in het geval van verzuim of verlies van inkomsten? Voor particulier verzekerden in de VS heet dit disability insurance. Dat omvat verzuim en inkomstenverlies: http://en.wikipedia.org/wiki/Disability_insurance
NB: Ziektekostenverzekering (in algemene zin) betaalt ziektekosten: artsen, opnames, medicijnen, behandelingen.

I think there is a comma missing. Three things are being dealt with: The application form, a formal record of absence from work under the Health & Safety laws and a record of earnings and benefits (the inkomen pakket). These are things which must be submitted by the applicant when applying for the insurance. So "Aanvraagformulier Verzuim" is not an expression in its own right, I suspect.
