I can't seem to find any of the institutions/bureaus that are abbreviated here. Thanks in advance for any help.

I did some serious digging on the web and found what I think you are looking for.
EVA = Externe Verwijzings Applicatie
VIS = Verificatie Informatie Systeem
SFH = Stichting Fraudepreventie Hypotheken

Now, as far as translating that into English, that's a totally different ballgame.
