This is a title of a document with fields like Name/address/contract-data/salary info/ etc for new employees in a company.

The first thing that comes to mind is [Employee Payroll Form] but maybe there is some official term?

Or change of employee details, or new employee information, or payroll information - there's loads of different ways you could translate it. If you're translating the form itself, you could leave out "form" as it's obvious.
