From a policy manual for UMC Utrecht:

"Het Universitair Medisch Centrum Utrecht (UMC Utrecht) is een organisatie met taken op het gebied van topklinische en topreferente patiëntenzorg, van wetenschappelijk onderzoek en onderwijs."

I've found out that topklinisch refers to clinical care that can only be performed by specially certified hospitals and that topreferent refers to special medical cases, but I haven't been able to find equivalent English terms.

Services is more common than care. 'Patient' can be left out, since it is obvious and would sound awkward in English. I always find the word 'top' a bit confusing because it sounds as if it refers to the quality of care rather than the type of care.
