Context:
Door de grote vullingsbehoefte die shock, zowel cardiogeen als septisch, veroorzaakt kan het normale gewicht van de patiënten met soms meer dan 20 procent van het thuisgewicht toenemen.

Also, what is thuisgewicht? Is it just another word for normal weight?

Explanation:
If I understand correctly, this refers to the lack and need of blood. However, it is broader than that. It is about the need so supply the patient with more body fluid in general:

"Gecombineerd met de lengte en het gewicht van de patiënt wordt aan de hand daarvan de vullingsbehoefte (vocht) automatisch berekend. Daarbij wordt rekening gehouden met onder andere de grootte van lichaamsdelen voor verschillende leeftijden."

A lack of body fluid can be solved by transfusion of blood plasma and/or blood cells. In general I would say transfusion need. For example in:

"Establishing Transfusion Needs in Burn Patients"

Thuisgewicht: I am not sure, but presumably simply the "normal" weight a patient has when outside hospital, or the weight before the problems started that brought him/her into hospital.

Explanation:
fluid replacement/resuscitation are used to describe the process of replenishing body fluids including blood, sweat and so on.

How about baseline weight for thuisgewicht?
