Dutch term or phrase: ponsplaatje
This term appears in a medical appointment letter for a pre-op consultation. The patient is advised to bring his/her targeted history form, the medicines he/she is taking and "ponsplaatje". I looked this up in a few sources and didn't find anything. The two root words didn't ring a bell. A European online multilingual medical glossary did not show this term. Does anybody know this term?

Explanation:
Ponsplaatje
Als u voor de eerste keer op de polikliniek komt, schrijft u zich in bij de centrale inschrijving. U vindt deze balie in de centrale hal van beide locaties. Hiervoor moet u meenemen:

bewijs van inschrijving bij een ziektekostenverzekeraar
geldig legitimatiebewijs (paspoort, rijbewijs of identiteitskaart)
Een medewerker van de centrale inschrijving neemt uw persoonlijke gegevens op en maakt daarvan een ponsplaatje. Daarna meldt u zich bij de receptie van de betreffende polikliniek. Het is van belang dat u bij elk bezoek aan de polikliniek, onderzoek- en behandelafdeling uw ponsplaatje en legitimatiebewijs meeneemt. Bij een volgend bezoek kunt u zich dus rechtstreeks melden bij de polikliniekbalie van uw specialist. Alleen als uw persoonlijke gegevens zijn veranderd, moet u dit weer doorgeven bij de centrale inschrijving. U ontvangt dan een nieuw ponsplaatje
