and (K.) means (at least according to normal convention) that the whooping cough component is optional

A quick google suggests that English usage is 'DPT-polio'.

My suggestion would be 'repeat DPT-polio/DT-polio', since it will be one or the other (the pertussis is optional in the sense that it is either included the combined vaccine or not included).

If you can afford to, I'd suggest waiting for confirmation or correction by the resident medical prozians
