Vanuit het XX zijn op basis van een Samenwerkingscontract (met een totale begroting van 84000 Euro) pilots voor veranderprojecten voorbereid op de genoemde probleemgebieden, namelijk betreffende *** zorgtrajectbegeleiding *** en vroegdiagnostiek. Het YY netwerk heeft ervoor gekozen zowel op de gesignaleerde probleemgebieden als de voorbereide projecten aan te sluiten en daarmee ook de succesfactoren uit het XX netwerk te benutten. Dit wordt ook uitgedrukt in het focus van het netwerk op complexe problematiek bij ouderen met specifieke aandacht voor cognitieve stoornissen en dementie.
Tijdens de al gehouden bijeenkomsten van ouderen in het kader van YY is de zinvolheid van dit thema en de reeds opgestarte projecten onderschreven
