Een zorgzwaartepakket (ZZP) is een manier om aan te geven welke zorg iemand krijgt die niet langer zelfstandig kan wonen. Het gaat om zorg die noodzakelijk is, en die zo goed mogelijk past. Wie deze zorg nodig heeft, krijgt de indicatie als een ZZP. Een ZZP geeft per soort zorg aan wat u krijgt, en hoeveel. De budgetten van zorginstellingen zijn gebaseerd op ZZP's.
