I am trying to keep the sentence in one piece, but how to handle the adverbial quality of "de afhankelijk van de ernst van de kwaal" verminderde?

I think this is how to deal with it - otherwise it still remains a very Dutch sentence

agree  Adam Smith: Probably the best option, but I'd get rid of "extent depending", and simply have "reduced BY the severity of disease", IMHO. :-)
