The radiograph was taken about 18 months after the fracture was sustained.

How would you translate 'onderbouw'? I thought of consolidation or union.

Also the verb 'doorbouwen' is used in another section: 'Fractuur voor een groot deel doorgebouwd'
Is it consolidate? (It is used here about 1 month after the fracture)

My guess is that 'doorbouw' refers to the remodeling phase of bone healing

1. Reactive Phase
i. Fracture and inflammatory phase
ii. Granulation tissue formation
2. Reparative Phase
iii. Cartilage Callus formation
iv. Lamellar bone deposition
3. Remodeling Phase
v. Remodeling to original bone contour

Fracture Healing Time

According to the X-rays, MRI and CT scan results, bone fracture healing time can take in different duration. Going into the details, the following will specify the duration of healing time.
• During the granulation stage, there will be formation of soft cartilage and this may happen 2 and 3 weeks after the injury.
• On the lamellar bone deposition phase, the formation of hard cartilage will happen at about 4 to 8 weeks after the injury.
• The bone remodeling phase starts between 8 to 12 weeks after injury has happened. The bone will slowly remodel the bone back to its original form.

I have tried to find some more information about 'doorbouw'

compare with the period mentioned above:

Gemiddelde tijd tot volledige doorbouw van de fractuur (bij röntgen controle) bedraagt 16 weken met een range van 12 tot 32 weken.

clearly, 'doorbouw' refers to the actual healing process, the forming of bone:

Van der Linden plaatst, twee maanden na de zuggurtung-fixatie, een hoekstabiele plaat. Ondanks de plaat blijft doorbouw achterwege en de vrees voor pseudartrose neemt toe.

I found (ntgv) a definition of 'doorbouw' in a pdf but it has no adress line, you can find it when you Google "fractuur" "doorbouw" third page

Wanneer er röntgenologisch voldoende doorbouw is - d.w.z. wanneer de architectuur van het verlengde bot vrijwel normaal is - wordt de fixatuur verwijderd

'normal architecture' would refer to the remodeling phase

agree  Barend van Zadelhoff: sounds good // Many thanks, Anne. Until now I saw only a reflection as in a mirror, but now I see face to face. :-) I considered ossification to be part of the remodeling but, with hindsight, this is obviously incorrect.
