case worker
Explanation:
I'm not sure if this is exactly the term you are looking for. It would depend upon the context. If it has to do with social work, than it's appropriate.

In social work, a caseworker is a type of social worker who is employed by a government agency, non-profit organization, or other group to take on the cases ... and follow them through.

sorry, it came out as two words, but I think it's supposed to be 'caseworker'.

I opted for 'case worker', despite Sindy's apprehensions, but many other suggestions would have worked too. I 'll include them in the KudoZ glossary. Thank you all for your help!

agree  Mark Straver: Either your term or Sindy's alternative would work well.

disagree  Carolyn Gille: to me a 'personal coach' is something rich people spend a lot of money on. A caseworker is something totally different. It's refering to the person who is responsible for following a person (usually in a government institution) from start to finish
