Is this some kind of document for "security clearance" or "health certificate" to be presented to employer?

Are you a contractor?
A clearance certificate is a document issued free of charge by the WSIB [Work Place Safety and Insurance Board]. It declares that you:
are registered with the WSIB, and
have an account in good standing.
A clearance certificate clears the person hiring you of financial liability and ensures they won't be held responsible for your WSIB payments.

All clearance certificates are valid for 60 days. Requests for new clearance certificates should be made one week before the expiry date of the current certificate to ensure continuous protection.

Are you hiring a contractor?

is registered with the WSIB, and
has an account in good standing.
More importantly, it clears you of financial liability and ensures you won't be held responsible for someone else's WSIB payments.

Clearance certificate


Explanation:
A clearance certificate is a legal document which certifies that any amount of money you are liable for (taxes etc.) have been paid. Signing certain contracts can make the other party liable for the amounts you are owing, that is why a clearance certificate is necessary.

Although "clearance certificates" can be needed in a wide range of circumstances, in this case, the Clearance Certificate is required in order to prove that the employee doesn't owe his last employer or company any money or items.
