Could somebody kindly explain what "at the origin" means here?

agree  Harry Borsje: Standard phrase is (e.g.): due to faulty materials or workmanship (origin = manufacturing process); however, origin here could perhaps also mean the original supplier to whom the piece has been returned under warranty; weird source either way
