relapsing-remitting multiple sclerosis
toenemend-afnemende veelvuldige sklerose

Explanation:
I'm not 100% sure about this, but this is how I would have translated it. Before 2002 I would have written "toenemende/afnemende" because that makes more sense to me, but the 2002 spelling rules prescribes the ambiguous hyphen. I guess you can also use "toenemende-afnemende". Another way to translate it may be "veelvuldige sklerose wat kom en gaan", I think.

Notes to answerer
Asker: Thank you for this suggestion. the term that medics seem to use is "terugval" and "terugvallende". However, your suggestion makes a lot of sense, except that toenmend-afnemend has a very different meaning to toenemende-terugvallende referring to two opposite conditions.

Asker: Further comment: "toenemend" implies the condition is getting worse. A relapse is when the condition flares up again after being in remission. Commonly referred to as "terugval". Any other comments?
