Explanation:
1. On "associate": The word "associate," as used in a company's workforce, can mean one of two distinct things:

- In some companies, it is used as euphemism for "worker" or "employee." The purpose is to impart a sense of empowerment, teamwork, and camaraderie. The idea is that the workers are more than just workers. They have a stake in the company, and their mode of operation is that of togetherness.

- In other companies, the word is used to distinguish a particular layer of the workforce, usually that which is below the top rung. For example, in law firms, the top rung consists of partners; the lower rung consists of associates. In this company, at appears that all employees below the "leadership" level are called "associates." Notice how the associates are being asked to comment on the activities of the leadership.

For all these senses, I find زميل and زملاء useful, although in some situations (also surveys), I used the expression منتسـبي الشركة. You may want to ask your client about the specific sense intended here.

2. On the word "experience": This is typically a tough one to translate to Arabic, simply because it is used in so many different ways. For instance, it is used in the sense of work history, in the sense of development of knowledge and skills, in the sense of a real-life encounter with a situation. Not all of these translate in the same way. In this particular survey, it seems to point to the "associate's" formed opinon or impression based on his/her real-life encounters.

One could say ستبيان also, but this would not give it an all inclusive implication of a study as the word مسح. The latter might entail more analysis, than merely just getting the answers to a questionnaire, whihc is the more appropriate translation for استبيان
